Rahm Kota was one of the few Jedi to survive Order 66.

Background

Rahm Kota was one of the first people to be suspicious of the clone soldiers. During the Clone War he was one of the only generals not to use clone soldiers, instead he recruited his own army to serve him during the war. After Order 66, many of his army stayed loyal to him, so he decided to attack key points in the Galactic Empire, although his plans were for naught when he was blinded and disgraced in battle by Vader's secret apprentice Starkiller.
